Thank you for your child's application to Head Start. To finalize your application please call the center you are applying to and make an appointment for enrollment.

#1. Birth Certificate of CHILD. (This must be original and not a copy.)

        -Also accepatable is foreign equavelant of birth certificate. Or hospital birth record (until birth certificate is obtained.)

#2. Proof of Income for Entire Household. (This may include one or more of the following documents.)

       -2018 Income Tax Forms

       -Employer W-2 Forms

       -Most current pay stub or pay forms

       -Written Statement from employer

       -Document showing current status of assistance. (TANF or SSI)

       -Document showing Foster Care status (of applying child)

#3. Child's Medical Insurance Card. (Or medicaid card)

#4. Child's Current Immunization Record.

        ​
