Scanned documents required

In order to complete your application and receive services, the following scanned documentation is required. Documents can be scanned using a desk scanner, or by taking clear pictures of the documents with a phone or tablet.

If your application is incomplete due to missing one of the following scanned documents, you application will take longer to process.

The following documents are needed to process your service request.

1.   Identity

  • Social Security cards for everyone in the household.
  • For anyone 18 years of age and older- Valid Government issued photo ID such as a Driver's License or Military ID
  • For anyone under 18 years of age- You may provide a medicaid card, School ID or Shot records.
  • ** If you do not have any of the documents above, please call the office for other acceptable documents**    

2.   Proof of any type of earned or unearned income received in the past 30 days for all household members. 

  • Employment wages (all check stubs for weekly, bi-weekly or monthly wages for the last 30 days.
  • Self-Employment wage documentation.
  • Social Security / Supplemental Security Income (SSI) award letter for 2020 (bank statements will not be accepted)
  • TANF benefits for 2020.
  • Unemployment benefit or workers compensation statement of benefits.
  • Private Pension Documents for 2020.
  • VA Retirement / Disability for 2020.
  • Child Support Statement or current payment listing form from Attorney General's Office.
  • Any other source of income.
  • Any checks received in the past 30 days for all household members. (Do not scan W2 tax forms)
  • Worker's compensation.

3.  If no income or partial income has been earned or received for the last 30 days for any household member, age 18 or older, you must provide a signed statement that you have no income for the period.  You must include a statement on how you have been supporting yourself and your family for the last 30-day period.

4.  If any member of the household is Disabled, please provide proof of disability.  You may provide a Social Security Award Letter. 

 Applications will not be processed without required documents listed above. Due to increased volume of applications, please allow up to 15 business days for processing.

 

 

Eligibility Requirements for all Programs

·       Participants must be a resident of either Catron, Cibola, McKinley, Socorro or Valencia Counties

·       Most programs require participants to be at a specific level of household income according to the Federal Income Guidelines.

 

Depending upon the number of requests, funding for some programs may be limited or unavailable.

What is TEFAP?

The Emergency Food Assistance Program, or TEFAP, is a federal nutrition program that moves food from farms to food banks to people facing hunger.

Through TEFAP, the USDA purchases healthy foods—fruits, vegetables and other staples—from U.S. growers and producers. Feeding America network food banks, and other food-distributing organizations, then partner with states to distribute this food to people in need.

The program is a cornerstone of the Feeding America network’s food supply. Last fiscal year, the Feeding America network received 1.24 billion pounds of food from TEFAP, providing over 1 billion meals to people facing hunger.

 

Mid-West NM CAP partners with Road Runner Food Bank through our local food pantries in Valencia and Socorro Counties.
